The new Sentara Community Health and Wellness Center located in the Sentara Martha Jefferson Hospital Downtown Outpatient Care Center will expand on existing services while incorporating new programs designed to meet community needs identified in the most recent health assessment.
Promoting Healthy Lifestyles
Downtown Food Pantry
Equipped with cold storage for fresh produce, the food pantry will serve as the distribution hub for the SMJH Free Fresh Farmstand Program, which extends food assistance to rural areas.
Exercise and Fitness Programs
Certified fitness instructors will lead free exercise sessions across multiple locations, including Charlottesville, Louisa, Gordonsville, and Esmont.
Enhancing Mental Health Services
Counseling Support
In partnership with The Women’s Initiative, the center will provide private counseling spaces and a shared reception area to improve access to mental health services.
Community Outreach
Programs will focus on mental health and substance abuse education in schools, as well as collaborations with local organizations for drug and sharps disposal events.
Advancing Health Equity and Access
Chronic Disease Management
A registered nurse will offer routine care, chronic disease management, and referral services in private exam rooms.
Educational Space
A multi-purpose classroom will facilitate free classes in collaboration with maternal health organizations and other community partners, addressing prenatal and postpartum care.
Supporting Workforce Development
Career Growth Initiatives
Workforce development programs, including "earn while you learn" opportunities, will help individuals pursue careers in healthcare.
Technology Access
The center will provide WiFi and computer access to assist job seekers with applications and employment resources.
The Community Health and Wellness Center is poised to become a vital resource, addressing key social determinants of health and expanding essential services. By enhancing existing programs and introducing new initiatives, we remain committed to fostering a healthier, more equitable future for all. This new facility will not only improve access to critical healthcare resources but also create an environment where individuals and families can thrive.
